By the sounds of it, you are so close!
If you have a data source created already containing all the information that you would like to be pushed into the form, then you will need to go into the Form Builder and create a Dropdown or Radio Button question (Dropdown would be better in your case) and then configure that dropdown question to push values into other questions.
In the Options tab of the dropdown question, the piece you are looking for is under the Options tab and all the way at the bottom. Here is our documentation about this feature: Populate Answers in a Regular Section and focus on the "Populate Answers in a Regular Section" section of the article for exactly what you are trying to do.

If I upload a Data Source with a list of customers that includes other cells of information, can I select a customer from a list in the form and have multiple fields then populated from other cells in that data source?
My application would be a contractor selecting a customer from a list, and then having their phone number, address, city/town, state, and work order number automatically filled in on the form all in a single step.
I have successfully added a data source, and I can reference it for information but I am struggling to populate multiple fields in this single step.
